Responsible for administering a health survey questionnaire in a client's home and community-based setting using a standard health questionnaire.  Task does not require any clinical assessments.

Primary Responsibilities:
  • Interact with assigned communities to locate members who cannot be reached by telephone by using last known address furnished by member's health plan
  • Proceed to member's home previously appointed through the Optum customer call center for the purpose of completing an in-home assessment.
  • Explain purpose of health assessment and persuade member to agree to complete a health assessment survey in member's home or on another appointed day/time preferred by member
  • Overcome member refusals by following standard operating procedures and scripts
  • Administer the health risk assessment within 30-45 minutes using a standard health assessment questionnaire and record responses on a hard copy form or an electronic form using computer
  • Interview residents of member's known address to attempt to obtain an updated contact information for member who no longer resides at the visited address
  • Directly reschedule member for another day and time if member cannot be persuaded to complete assessment survey on appointed day or drop-in visit day
  • Record outcome of each appointed visit or drop-in visit following established standard operating procedure

Requirements

Required Qualifications:
  • High School Diploma/GED.
  • Comfortable working up to 36 hours per week, Monday - Saturday between hours of 8:00am - 5:00pm.
  • Comfortable traveling 100% of the time.
  • Reliable transportation, a valid CA driver's license, and auto insurance.
  • Must have personal computer at home along with internet connection.
  • 1+ year work experience in Community Outreach, Social Work, Customer Service, Sales or Healthcare interacting with geriatric and/or disabled populations.
  • Must be comfortable using a computer and Microsoft Outlook.
  • Knowledge of the assigned territory.
Preferred Qualifications: 
  • Understanding or familiarity with Medi-CAL and Medicare populations.
  • Familiarity with the Healthcare field/environment such as Medical Clinics, Hospitals, Health Insurance.
  • Microsoft Excel experience.
  • Microsoft Word experience.
  • Bilingual with English and Korean or Chinese.
