This is a full-time work at home role, but you must be located in our West Region: specifically in Utah or Texas. This is a new position to the department and you will make a difference in the quality of life of the members you work with.
POSITION SUMMARY
Behavioral Health Specialist provides consultation and collaboration to medical nurses in case management program (post-hospital and community care), regarding engagement and treatment of BH co-morbidities. The mission is to decrease readmissions through implementation and coordination of holistic approach to psychosocial, BH and chemical dependency issues. Provides psychosocial interventions through resource identification, program development and other means. This position serves as a consultant to case managers, social works, pharmacists and physicians along with maintaining a small caseload of intensive BH cases.
Fundamental Components:
- Provides consultation to nurses, working on medical cases, in regards to the assessment and awareness of BH co morbidities.
- The BH Specialists leverages clinical assessment skills and treatment planning to coach and collaborate with the nursing staff to include BH co morbidity information in their work, with the patient, to facilitate the single-point-of-contact model.
- Supports nursing staff with engaging members using motivation interviewing skills.
- Performs and monitors member impact and outcomes, as a result of this integration and collaborative process.
- Maintains contact and outreach with nursing staff in order to adjust treatment interventions, etc. to achieve the desired results.
-Conducts regular discussion and updates with behavioral health providers, primary care physicians, Medical Directors, pharmacists, and care management staff regarding the status of particular patients.
-Serves as a member advocate to ensure the member receives all of the necessary care allowed under the members benefit plan.
-Understands healthcare reimbursement methods that promote the provision of cost effective healthcare and the preservation of member benefits.
-Utilizes community resource expertise and alternate funding arrangements available to plan members when services are not available under the benefits program.
-Outreaches programs as appropriate to reduce admissions for acute and chronic members and assist with decreasing inpatient lengths of stay, and preventing avoidable emergency room utilization.
BACKGROUND/EXPERIENCE required:
3-5 years of direct clinical practice experience, master's degree.
minimum of 3 years experience with medical social work or nursing in appropriate setting with Behavioral Health clients
Ability to travel locally as needed (within 50 miles).
BACKGROUND/EXPERIENCE desired:
Managed care/UR experience preferred.
EDUCATION
The level of education desired for candidates in this position is a Master's degree.
