GENERAL PURPOSE OF THE JOB:

This position functions under the clinical direction of a Registered Nurse to assist in providing day-to-day leadership and direction to patients and teammates in the transition of patients from central venous catheters to AV fistulae or AV graft. Position is the source of Vascular Access information and care coordination for designated facility(ies) and compiles and maintains Vascular Access Data to ensure accurate tracking and trending.

ESSENTIAL DUTIES AND RESPONSIBILITIES :

The following duties and responsibilities generally reflect the expectations of this position but are not intended to be all inclusive.

In collaboration with the facility's RN-CV Catheter Specialist, Charge Nurse or Clinical Coordinator and under the clinical direction of either, coordinate the care and management of patients' Vascular Accesses at assigned facility(ies)

Ensure a plan is developed for each patient with central venous catheter to transition to AVF fistulae or AVF graft

Coordinate and facilitate referrals necessary for all steps of permanent vascular access placement (vessel mapping, surgical evaluation, permanent access placement surgery)

Collaborate with hospital and acute staff when patient is admitted to a hospital to coordinate necessary follow-up regarding vascular access plan

Coordinate scheduling of procedures and follow-up communication between nephrologist and surgeon's office for patients

Under the direction of RN, Charge RN, Clinical Nurse Manager, or Clinical Coordinator, make and/or ensure appointments are made and completed for each step of the process and that central venous catheters are removed timely; record removal date accurately in Snappy

Coordinate care of new dialysis access AV Fistula/Graft (including, but not limited to scheduling and following-up on maturation evaluation and first cannulation)

Collaborate with the Multidisciplinary Vascular Access Team to facilitate decrease in patients' dependency on central venous catheters and track patient progress from central venous catheter to AV fistulae or graft

Proactively collect and report to the Multidisciplinary Access Care Team accurate and complete data and assessments of the facility, teammates, provider partners, and hemodialysis patients for baseline and ongoing status of vascular access types

Coordinate Vascular Access-related communications between Vascular Surgeons, Interventionalists, and all Dialysis Care Team Members to secure and disseminate follow-up reports

Maintain accuracy of and update access plan tracking tool as necessary

Participate in Continuous Quality Improvement of Vascular Access planning to improve patient and facility outcomes; actively seek areas of improvement opportunities

Comply with DaVita policies and procedures in accordance with all state and federal regulations for patient care and services (CMS, HIPAA, OSHA, or other governing agencies)

Comply with state and federal regulations of the Injury Prevention and Safety Training Program Perform other duties and responsibilities as assigned

Able to work overtime with little or no notice as needed

Attend staff meetings, phone conferences, and training as needed

Know facility emergency plans and demonstrate ability to act swiftly, confidently in an emergency

Know, understand, and follow teammate and employment policies and procedures

Consistent, regular, punctual attendance as scheduled is an essential function

MINIMUM QUALIFICATIONS:

(Education, licenses, certifications, and experience required to fulfill the essential duties, include computer skills as required):

High school diploma required

Minimum of two (2) years' experience as hemodialysis patient care technician required

Current CPR certification required (or must be obtained within 30 days of employment)

Certified Hemodialysis Technician (CHT) certification preferred; certification must be obtained within 18 months of employment

Meet all applicable state requirements for education, training, credentialing, competency, standards of practice, certification, and licensure in the State in which employed as a dialysis technician (AKA: patient care technician) required

Intermediate computer skills and proficiency in MS Word, Excel, and Outlook required; proficiency in all DaVita clinical computer systems/software required within 90 days of employment
