Essential Functions & Responsibilities
Ensures quality and safe delivery of services to patients according care plan. Uses equipment and supplies safely, effectively, and efficiently.
Provides patient, family, and/or caregiver education and information pertinent to diagnosis and safety issues.
Maintains accurate and timely documentation of clinical records and time and travel entries.
Integrates and coordinates the care of the at-risk population across the healthcare continuum.
Services as a patient advocate, educator and as central source of communication for the patient and their support systems, and referral sources.
Ensures quality and safe delivery of services to patients according to care plan. Uses equipment and supplies safely, effectively, and efficiently.
Promotes the provisions of comfort care to the dying as an active, desirable, and integral component of nursing care in a hospice facility.
Communicates effectively and compassionately with the patient, family, and hospice team about end-of-life issues.
Uses scientifically based standardized tools to assess symptoms (eg., pain, dyspnea, constipation, anxiety, nausea/vomiting, and altered cognition) that are expected by patients at the end of life.
Assesses and treats multiple dimensions including physical, psychological, social and spiritual needs to improve quality of life.
Qualifications/Experience
Graduate of State Board approved program for Registered Nurses and valid license as a Registered Nurse.
CPR: Maintain a valid Basic Life Support (BLS) Healthcare Provider Card with Re-certification.
Strong interpersonal skills.
Ability to work as a collaborative, inter-disciplinary team member.
Valid drivers license with automobile insurance in accordance with state and/or organizational requirements.
