RN Case Manager - Emergency Department

BSHSI POSITION SUMMARY The primary responsibility of the Clinical Resource Manager is to provide coordination of care through facilitation/communication and discharge planning throughout the continuum of care. JOB REQUIREMENTS 1. A Clinical Social Worker or Licensed Registered Nurse, Bachelor's and/or other Advanced Degrees preferred.2. Work requires the employee have five years of clinical experience with two years case management coordination preferred.3. The employee must have high level communication and organizational skills, and be able to work independently. An essential function of this position is to comply with BSHSI Policy (SYS.HR.NUR.001) which requires, as a condition of employment, that all registered nurses with diplomas or associate degrees who apply and are hired after May 1, 2015 complete a Bachelor of Science in Nursing (BSN) degree within five years of start date.
