Description

REM Minnesota Community Services is seeking a Program Coordinator to provide assistance to individuals with disabilities in a group residential setting. Program Coordinator duties include:

  • Facilitating the implementation of Individual Service Plans (ISP)
  • Participation in the development of Risk Management and Assessment Plans (RMPs)
  • Participation in staff hiring, training and scheduling
  • Oversight of all the day to day operations of an adult foster care site
  • Serving as a link between our direct care and clinical staff to ensure a seamless delivery of services
  • Providing direct care supports to individuals receiving services, such as assistance with daily activities, medication administration, and personal care.
  • Accompanying individuals on outings in the community

Qualifications

Requirements:

  • High school diploma or GED
  • Three years related work experience
  • Must be 18 years of age
  • Valid driver’s license in good standing
  • Car registration and vehicle insurance if providing transportation for individuals receiving services
  • Successful clearance of local and/or state background checks
  • Supervisory experience preferred
