DescriptionREM Minnesota Community Services is seeking a Program Coordinator to provide assistance to individuals with disabilities in a group residential setting. Program Coordinator duties include:
- Facilitating the implementation of Individual Service Plans (ISP)
- Participation in the development of Risk Management and Assessment Plans (RMPs)
- Participation in staff hiring, training and scheduling
- Oversight of all the day to day operations of an adult foster care site
- Serving as a link between our direct care and clinical staff to ensure a seamless delivery of services
- Providing direct care supports to individuals receiving services, such as assistance with daily activities, medication administration, and personal care.
- Accompanying individuals on outings in the community

QualificationsRequirements:
- High school diploma or GED
- Three years related work experience
- Must be 18 years of age
- Valid driver’s license in good standing
- Car registration and vehicle insurance if providing transportation for individuals receiving services
- Successful clearance of local and/or state background checks
- Supervisory experience preferred
