Care Manager
The Care Manager (CM) is the core of the Assisted Living business. He/she supports our residents by providing personal care, housekeeping, and culinary services. In Life's Neighborhood (our memory care program) this person also provides the bulk of all resident activity. The CM will utilize the principles of choice, respect for individuality, dignity, privacy and independence when caring for our residents. He/she works with all members of the team and is expected to be aware of changes in the residents' health or mental status.
General accountabilities:
"Provide for the daily physical care of residents. Assist residents in getting up, toileting, washing, showering, dressing, grooming, brushing teeth, and with other personal hygiene and assistance needs.
"Provide an environment that enhances the psychological well being of residents.
"Assist and serve meals to residents.
"Clean up the dining room and set up for the next meal.
"Review the assignment sheets and service plan for each resident's care and ensure completion.
"Interact with family members and others on care issues.
"Notes changes in a resident's condition on a day-to-day basis and inform management staff of changes.
Knowledge, Skills, Qualifications:
"Experience working with older adults preferred
"Experience working with persons with dementia a plus
"Caregiving experience in Home Care or Assisted Living strongly encouraged
"Able to communicate effectively with seniors who have multiple communication barriers
