The Clinical Social Worker provides crisis intervention, short term counseling, information and referrals for patients and their families, ranging in age from infants through geriatrics. Provides coordination of complex discharge planning needs and concrete service needs of patients. Leads educational and support groups. Provides consultation related to psychosocial aspects of care as a member of the interdisciplinary treatment team. Serves as an expert in community resource programs. Is a liaison to community referral agencies, both public and private. Participates as a member of the Bioethics Committee.

The Clinical Social Worker coordinates all aspects of social barriers to care with the case manager and the interdisciplinary team, to ensure effective, high quality, timely and cost-efficient results.

The American Case Management Association places the role of Social Work in acute care hospitals in the realm of Case Management and describes the functions as follows:

"Case Management (Social Work) in Hospital and Health Care Systems is a collaborative practice model including patients, nurses, social workers, physicians and other practitioners, caregivers and the community. The Case Management process encompasses communication and facilitates care along a continuum through effective resource coordination. The goals of Case Management include the achievement of optimal health access to care and appropriate utilization of resources, balanced with the patient's right to self-determination."

Serving the greater Stanislaus County with 423 plus acute care beds, Memorial Medical Center provides a full continuum of care including 24-hour emergency care, inpatient and outpatient surgery, obstetrics, pediatrics, nuclear medicine, dialysis therapy, diagnostic imaging, intensive care, physical therapy, and home health. Specialized care includes Memorial Regional Cancer Center and Memorial Heart Institute. Minimum Qualifications:

Masters Degree in Social Work from an accredited school of social work.

One (1) year of clinical social work experience in an acute care, home care, or SNF OR Masters Degree program field placement in a health care setting may be substituted for one (1) year of experience.

Proficient in using a computer to accurately enter and extract data, send and receive email, calendar appointments, and use task lists as will be using a variety of computer software programs.

Ability to manage own time and schedule own tasks. Must have initiative to work efficiently without constant supervision and direction.

Ability to work effectively with other disciplines.

Must demonstrate excellent written/verbal interpersonal communication and problem solving skills.

Ability to read, write, hear and verbally communicate in English to the degree required to perform the job.

Must be able to travel in accordance with any requirements of your job (if MMC and/or own vehicle used on MMC business, must possess valid California Driver's License and upon request proof of liability insurance).

Preferred Qualifications:

Possession of a current license as a Licensed Clinical Social Worker from the state of California.

Recent Clinical Social Work experience in an acute care hospital.

Experience with Microsoft Office products.

Bilingual, Spanish and English.
