RN Care Manager

Providing skilled nursing, medical and rehabilitation therapy, PowerBack Rehabilitation offers highly personalized care plans designed to get patients home sooner. Guest Services Teams see to each patient's needs providing the services and amenities found in better hotels. This allows our nursing teams to focus solely on our patients' progress. As a RN Care Manager, your responsibility to patients' progress is through the delivery of high-quality and cost-effective health care while achieving positive clinical outcomes. In addition, patient/family and employee satisfaction are priorities. The RN Care Manager leads the Primary Care Teams and is responsible for the care management of the patient's assigned to those teams and for providing the physical care that requires the expertise of a Registered Nurse. Also, you will assume some administrative functions. As such, the RN Care Manager is required to possess clinical expertise for the patient population, strong communication, mentoring, delegation and coordination skills, and the ability to work with physicians and other medical professionals. The RN Care Manager ensures a high performing team by organizing and prioritizing activities of the members. NRNC

Qualifications:

SPECIFIC EDUCATIONAL/VOCATIONAL REQUIREMENTS: 1.Must be a graduate of an accredited school of nursing, college or university with a current registered nurse licensure by the State Board of Nursing. 2.A minimum of two years full-time or equivalent clinical experience is required and a minimum of one years of clinical experience in long-term care nursing is preferred. 3.BSN preferred EEO/AA, M/F, Vet, Disabled
