Job Type: Full Time Position Summary:

In this role, you will be the primary patient contact for assessments, education and questions. You will manage treatment guidelines by placing outbound calls to patients and healthcare professionals to review CVS Caremark services, verify compliance and build supportive relations. You will review assessments and enrollment documents to ascertain eligibility for certain programs or infusions. You will also be responsible for, but not limited to, the following:

* Interact with various types of team members: Rph, Customer Service, Verifier and Technicians.

* Answer clinical questions related to drug-therapy and provide educational materials.

* Manage the patients in HBS and other systems by an initial assessment and follow-up tasks.

* Influence quality of care and clinical outcomes by reviewing patient profiles, monitoring for side effects, communicating with patients and other related parties as well as referring to other appropriate departments.

* Identify order specifications and clarify special requirements or actions to be taken with patient and Pharmacist.

* Assist pharmacy staff with questions relating to clinical information.

* Notify healthcare professionals of adherence concerns and possible side effects or adverse events.

Required Qualifications:

Candidates must possess excellent verbal and written communication skills in addition to the following qualifications:

* Clinical knowledge and analytical skills that will enable the identification of drug interactions, and potentially dangerous side effects which meet with medically recommended solutions is required.

* Advanced knowledge and experience with specialty medications and disease states.

* Attention to customer focus, ensuring that both medical professionals and patients are responded to with accurate and timely information.

* Excellent communication, customer service and interpersonal skills to address a diverse customer group.

* Attention to details, organization and accuracy.

* Ability to work in a team environment, providing leadership to enable timely completion of assigned work requirements.

* Demonstrated ability to go above and beyond in the service provided to the customers and/or patients

* Ability to handle mid-level customer service issues

* Ability to manage multiple tasks simultaneously

* Analytical and problem solving skills. Preferred Qualifications:

To be considered for this position, candidates must have excellent communication skills and telephone etiquette along with good technology skills and familiarity with Outlook, Word, Excel, etc., and ability to manipulate databases. Infusion pump experience preferred along with an understanding of and preferable experience in working with patients with LSD, HAE, PAH, Hemophilia IViG in addition to any other specialized conditions.

Education:

BSN preferred from an accredited School of Nursing. Licensed Registered Nurse in good standing in the State of Pennsylvania.
