About this job

1. Adheres to all Brookwood Home Health (BHH) policies and procedures and maintains standards of practice and documentation that are consistent with State and Medicare regulations and accreditation standards.

2. Completes and submits paperwork and visit notes as procedure dictates.

3. Provides updates and reports to, or consults with, the RN or PT Case Manager and/or the Director of Nursing on a frequent and as needed basis.

4. Provides direct and indirect care to the patients in the home setting, as contained in the written assignment on the Home Health Aide Care Plan and as assigned by the Registered Nurse, or appropriate therapy personnel. Personal care duties include assistance in:

a. Bathing in bed, tub or shower

b. Care of hair: shampoo

c. Shaving

d. Care of the teeth and mouth

e. Care of nails: cleaning and filing only (no cutting)

f. Toileting: bed pan, commode or toilet

g. Dressing

h. Physical Transfer

i. Walking/Ambulation

j. Marketing

5. Collaborates to implement the nursing and medical plan as taught by a

health professional and as directed for a specific patient:

6. Performs light housekeeping duties in the patient area:

High school or equivalent required. Must be a CNA verified through the Alabama nurse aide registry. One year experience. Home health preferred. Knowledge of Medicare coverage criteria for therapy services to Home Care Patients. Ability to audit charts for documentation, QI and UR purposes. Current driver's license, reliable transportation and auto liability insurance.
