Conifer Health Solutions is currently hiring for a RN Case Manager in Phoenix, AZ.

Overview: Under the direction of the Manager, Case/Disease Management, provides direction, identifies health and psychosocial issues, generates care plans and evaluates outcomes on members identified for high risk case and disease management.

Identifies issues relating to members' physical and mental well being that affects health status and works in collaboration with both the member and provider to determine appropriate care for case management services.

Maintains and documents initial assessment, and designs a care plan that is attainable, in the appropriate business application program.

Acts as a liaison between physician and member to alleviate potential issues that can interrupt care.

Communicates new information obtained through training, out-side courses, lectures, programs, etc. to peers for educational purposes.

Maintains case load by identifying cases that are no longer high in intensity or services and closes them timely and appropriately.

Motivates member to identify goals and works toward meeting them in a specified time based on all external factors.

Maintains and promotes confidentiality of patient information and adheres to all HIPAA regulations.

Participates in monthly case studies in Quality Management Department.

Special Skills:

Knowledgeable of community resources. Good computer skills, knowledge of medical terminology, good interpersonal communication and customer skills. Types 40-45 words per minute. Bilingual Preferred.

Minimum Requirements: Must hold an active RN Arizona License. Minimum of one to two years of case/disease management experience
