RN - Case Manager, Full Time (2416)

The primary purpose of the Case Manager position is to provide ongoing support and expertise through comprehensive assessment, planning, implementation, coordination and overall evaluation/monitoring of individual patient needs. The overall goal of the position is to enhance the quality of patient management and satisfaction, to promote continuity of care and cost effectiveness through the functions and integration of case management, utilization review and management and discharge planning.

Position requires working rotating weekends.

Education: High School Diploma or GED and a miminum of Associate Degree in Nursing.

Experience: Arizona Registered Nurse with a minimum of 2 years clinical experience. Minimum of 2 years experience in Case Management/Utilization Management required for Per Diem positions.

License/Certification: Valid Arizona RN license, BLS within 60 days of hire. If required to drive on company time, must have valid Drivers License and Proof of updated insurance.
