Medical Social Worker, Case Management (FT) - Full-time / Part-time

The Medical Social Worker is accountable to provide crisis intervention, biopsychosocial assessment, counseling, and complex discharge planning services for a selected population. Offers structure and stability to patients and their families to reach optimal level of understanding and acceptance of their illness . Provides consultation related to psychosocial aspects of care as a member of the interdisciplinary treatment team . Serves as facilitator for community resource programs and is a liaison to community referral agencies, both public and private . Masters Degree in Social Work from an accredited school of social work. LCSW preferred. One (1) year of clinical social work experience in an acute care, home care, or SNF OR Masters Degree program field placement in a health care setting may be substituted for one (1) year of experience. Previous experience with psychosocial assessment and crisis intervention. Recent Clinical Social Work experience in an acute care hospital preferred. Working knowledge/experience in utilization management, managed care and payer issues.
