Optum Complex Care Nurse Practitioner $5k Sign on Bonus - Yavapai County, AZ - Full-time / Part-time

This position provides primary care to a small, consistent caseload in addition to floating coverage for other providers on PTO or out on extended medical or family leaves. There is opportunity to participate in the on-call rotation which offers an attractive stipend (in addition to base salary) for each on-call shift worked (premium bonus for holiday call shifts). This is a Full Time  position and will include travel throughout the Yavapai County area, with mileage reimbursed at IRS allowable. Primary Responsibilities:

Providing primary care to chronically ill residents in long-term care facilities
Collaborating with primary care physicians, facility staff and patients' families
Participating in care conferences with other providers and patient family members
Educating nursing home staff, physicians and administration
 
Required Qualifications:

National Nurse Practitioner Certification in Adult, Family or Gerontology
Licensure in the State where this position is based (Arizona)
DEA license
Preferred past experience working in a nursing home or with seniors in other settings
Intermediate level of proficiency in PC-based word processing including basic Microsoft Word and Outlook skills
Preferred Qualifications:

Clinical experience
Long-term care experience
Knowledge of mental health issues
A background in managed care
Familiarity with Medicare and ICD-9 coding
