About this job

United Healthcare's Health Homes services include comprehensive care management, care coordination, care transitions, referral management, health promotion services, individual, family and caregiver support services, with services connected by health information technology.   This manager will oversee a team of Health Home Program Managers and Transformation Consultants.
 
Primary Responsibilities:

Develops and executes strategies for Health Home Program Managers in Iowa based on state mandates
Demonstrates understanding impact of a large business unit or multiple sites
Implement processes and programs to shape working relationships with internal and external customers
Gather feedback from internal and external business partners regarding clinical operational performance and effectiveness
Provide feedback to internal and external business partners regarding financial and quality metrics
Work with internal and external business partners to identify, define and implement opportunities to improve clinical programs and performance through Quality Improvement Methodologies
Meet with internal and external business partners regularly to provide up to date data on clinical performance and guide decision-making
Work with internal and external business partners to gain awareness of all issues and concerns
Attend all meetings as required (e.g., Joint Operations Committees for all practices, Health Home Program Manager meetings and Iowa  Health Plan meetings, Accountable Care/Health Home Central Regional, and National meetings etc.)
Translate into understandable information data related to performance and metrics
Works with Transformation Consultant to provide Clinical Performance data and metric evaluation of Health Home performance
Provide input into Central Region proposals for program improvement and outcomes
Ensure relevant continued training for Program Manager staff
Coordinate with local hospitals and community agencies for input to clinical care needs of the population
Facilitate case conferencing as needed with the entire Health Home Program Manager Staff to ensure that all aspects of the patient plan of care are followed, collaborate with CMO and clinical teams as needed
Review Population updates to identify any avoidable  ER visits or Hospital Admissions- 30 day readmissions for Health Home patients and ensure safe care transitions for patients
Troubleshoots and solutions with Program Managers on clinical issues that create barriers to care
Consult with the entire care team to identify and address barriers to care
Establish plans of correction that include interventions and monitor and evaluate outcomes of the corrective action
Provide reports for presentation to internal and external business partners
Consult with hospital teams Health Home Program Managers and practice team for Care Coordination across the population
Use the Accountable Care Population Registry/UHC Care Transitions to monitor care for high risk patients, plan visits, track follow up and ensure open care opportunities are addressed
Participate in clinical team huddles, operations reviews with Transformation Consultant and monthly JOC meetings with community based organizations, practice and hospital leaders to report progress on all clinical activities
Provides guidance to analyst when downloading eligible members for Health Home enrollment and assists with Health Home assignments
Monitor and manage the regulatory requirements of the program

Requirements

Required Qualifications:

Iowa State Licensed Professional Registered Nurse
3+ years of management experience
5+ years' experience working in the healthcare industry
Experience managing change initiatives for health care teams
Strong clinical data analytic skills
Strong process improvement skills/experience
Ability to organize and thrive in a fast-paced environment
Strong computer skills: Excel, Word and Visio
Preferred Qualifications:

Master's Degree 
5+ years of Behavioral Health experience preferred
3+ years' experience caring for patients with chronic conditions preferred
1+ years as a Care Coordinator working within a practice as part of the clinical team preferred
CPHQ certification
Six Sigma / Lean certification
Strong persuasion skills and demonstrated evidence of innovative thinking and program development to make the health care system work better
Experience in community based programs
