Position Summary

Performs direct and indirect care in collaboration with team leaders including, but not limited to LPNs, RNs, Charge Nurse

Minimum Education Requirements

High school diploma or GED

Graduate of accredited certificaiton course

Minimum Experience Requirements

Previous experience working in a senior community preferred

License/Certifications :

CO Certified Nurse Aide (CNA)

Cardiopulmonary Resuscitation (CPR) required, Basic Life Support (BLS) from American Heart Association required for facilities with an Automated External Defibrillator (AED) preferred for facilities without an AED.

Position Duties:

Patient Care:

Provides quality holistic patient care within scope of training including comfort and hygiene measures, preventative skin care, safety measures, transport, and nutritional and elimination needs in collaboration with the RN partner. Promptly answers call lights and responds to patient needs. Performs a variety of technical tasks to assist the RN to include the set-up, handling and usage of certain equipment such as: incentive spirometers, Foley catheters, glucometers, pulse oximeters, feeding tubes, and drainage tubes, Provides assistance with feeding, meals, passing water and snacks to ensure that patient's dietary needs are met. Collects specimens and handles bodily fluids practicing universal or other indicated precautions as directed by the RN. Reports patient status changes and patient needs to RN accurately and timely.

Ancillary Duties:

Performs other administrative documentation as required by unit (e.g. order entry, work logs, audits). Ensures that work environments and patient rooms are neat and orderly to enhance unit and hospital image and safety. Performs all duties in a conscientious, courteous and caring manner to maintain positive customer relations, and contribute to our Planetree philosophy.

Documentation:

Accurately charts patient documentation to include vital signs, I & O, percentage of meals taken, personal hygiene, weights or other procedures performed, and report findings promptly to RN. Organizes and prioritizes work to be completed within assigned time frame to promote consistent patient care and comfort.
