Overview:
Overview
Provides day- to-day patient care in the home setting which includes, but is not limited to: case management and total care of multiple patients with different levels of acuity, admissions to home care services, post-hospitalization assessments, recertifications, discharges, patient teaching, dressing changes, IV therapy administration, access site care and general skilled nursing according to policy and procedure, assists with other needs that may be apparent in the home to assure safety and appropriate care i.e. lifeline, meals on wheels, and instruction on public programs availability. Provides apropriate and timely documentation by utilization of the Epic computer system.
Per Diem position provides visits on a as needed basis following all department policies. Performs duties and responsibilities in a manner consistent with our mission, values and Mercy Service Standards.

OASIS data sets are accurate and done in appropriate time frame.
Admission assessments, post-hospital assessments, and recertifications are complete, correct, and timely.
Medication reconciliation is performed and profiles are complete and up to date.
Documentation is clear, concise and meets all regulations and standards.
Verbal orders are written promptly following the readback rule, care plan changes are instituted and care is initiated only with a physician's order
Pain management is facilitated through physician contact and patient teaching
Knows and practices infection control policies at all times:
Uses appropriate hand hygiene
Follows sharps policy, uses safety needles and other safety devices in performance of duties/procedures
Uses appropriate barriers and personal protective equipment in the home per policy
Maintains clean and dirty areas in the car
Performs and teaches proper disposal of home generated waste
Transports all laboratory specimens according to policy.
Reports lab results in a timely manner according to policy
Reports safety issues or injuries promptly
Maintains accurate supply utilization records and submits to office on a weekly basis
Maintains accurate mileage records and submits to office on a weekly basis.
Knows and practices infection control policy: patient contact equipment, bilibeds, telehealth monitors
Uses cameras effectively and submits pictures for printing on a timely basis.
Accepts assignments in a professional, caring manner
Accepts constructive criticism; uses situations for personal growth
Is adaptable to change: changes in daily schedule, changes in work schedule, changes in departmental
policy
Gives criticism/suggestions in a positve, constructive manner
Adheres to Mercy Values in all aspects.
Uses laptop computer in the home for assessments and for concise information gathering
Carries laptop in computer bag and cares for and maintains it in clean, working condition
Protects laptop form extreme heat or cold temperatures
Downloads computer nightly per policy to insure appropriate sharing of information and continuity of patient
care.
Demonstrates proficiency in clinical skills:
Performs throrough head to toe assessments which include vital signs, thorough cardiiopulmonary assessments, pulse oximetry, recognizes changes and reports appropriately
Performs wound care/dressing changes on all types of wounds using appropriate technique per physician order
Performs urethral and suprapubic catheterizations on male and female patients using appropriate technique
Performs newborn assessments, including all elements, utilizes appropriate equipment
Knows and uses normal growth pattern, developmental stages of infants and children and reports deviations promptly
Knows and uses age appropriate pain assessment tool
Knows pediatric medication dosage calculation and administration of medications using appropriate equipment/devices.
Understands and utilizes care planning, teaching and treatment of special needs children
Understands phototherapy and utilizes beds effectively
Obtain blood samples using appropriate device
Understands nutritional needs of pediatric population;knows and teaches appropriate breast/bottle feeding techniques.
Attends 90% of mandatory weekly staff meetings, unless scheduled for day off.
Participates in department and hospital quality improvement programs
Attends minimum of 70% of scheduled department inservice education classes.
Utilizes the Kronos Timekeeper System effectively and efficiently.
Qualifications:
Qualifications
Education: Graduation from accredited school of nursing.
Licensure: RN license in Arkansas
Valid driver's license in Arkansas or Oklahoma with good record
Automobile liability insurance in amount required by law in the state in which the automobile is registered.
Experience: Three years' nursing experience with minimum one year in a hospital setting, then preferably in home care or the equivalent.
Per Diem position requires minimum one year Home Health experience.
Certifications: CPR biannually
Other: Must be able to lift 50#
Must be able to transfer a patient weighing at least 100# with or without assistive devices
Must be able to bend, reach, stoop, climb stairs, push and pull
Must be able to stand, walk, and sit for extended periods of time
Must be able to drive at least 50 miles at a time
Must be able to drive in adverse weather conditions
Must have good visual or aural acuity, natural or corrected
Must be able to comprehend and follow instructions
Must be able to perform complex tasks
Must be able to learn the computer program currently in use
Must be able to relate to the public, particularly with the elderly
Must be able to maintain work pace relative to workload
Must be able to handle stress and change
Must be able to learn the CMS Rules and Regulations for Home Care
