Case Manager - Milestones Case Management Program

The Case Manager will assess and interview TANF recipients to obtain necessary information and background for accurately assessing their individual situations. The incumbent will identify the barriers keeping the participant from obtaining gainful employment and assist clients to resolve the barriers identified. The Case Manager is responsible for conducting all interviews and assessments in a professional and timely manner and in such a way that the rights and dignity of the clients are preserved. In addition, the incumbent will develop Individual Responsibility Plans to obtain employment, and motivate clients in carrying out that plan. The Case Manager will evaluate participant progress and initiate conciliation if necessary. The incumbent will report significant changes in TANF recipient situations to appropriate technicians, conduct group orientations and such instructional classes as required.

Work with a team of professionals and help ensure we make the best use of our resources to help achieve our mission. To be a good fit for this opportunity we need you to complete all necessary documentation, e.g., incident reports, program plans, charting, quarterly and final reports. It is critical that you maintain complete and accurate records and statistical data as required for all programs.

As an Case Manager, a candidate for this position may be disqualified if they have a criminal record for illegal drug use or sale, financial fraud, identity theft, physical abuse, crimes involving violence and/or theft. We do consider the specifics of the offense, including how long ago the offense took place. Valid Colorado Driver's License required.

What Else You'll Bring to the Table

· Bachelors Degree in Human Services or a related field and experience in a social service, counseling or rehabilitation setting.

· Excellent written and verbal communication skills Previous supervisory experience

· Must be flexible and professional at all times.

· Ability to relate professionally and effectively to fellow employees, DHS staff, and program participants.

· Experience speaking effectively to various small groups in presenting/informative setting.

Your specific activities will include:

· Must have the ability to work independently, cooperatively and to exercise effective planning.

· Demonstrate flexibility within the framework and pressure of established deadlines

· Must be able to write clear, concise and accurate reports and plans, submit them in a timely fashion and possess excellent time management skills.

· Have a working knowledge of community resources, have the ability to program, train and be creative in motivating program participants toward individual success.

· Conduct self in a professional manner and maintain a professional appearance that enhances the firm’s reputation.
