We are currently seeking a full-time Day Habilitation Trainer/Transportation Aide who will assist the Day Habilitation Specialist in their assigned core room or discipline with the implementation of individual treatment plans, participant supervision during non-goal planned periods and ongoing development of a habilitative program environment. The Day Habilitation Trainer/Transportation Aide is also responsible for assisting in the safe transportation of passengers in transit to and from the program site. The duties and responsibilities include, but are not limited to:

* Implement goal plan derived from the person's valued outcome as identified in their ISP and data collection to monitor progress.
* Transport individuals to volunteer and recreation activities in the community.
* Supervise, train and support individuals during outings in the community.
* Maintain accurate daily participant records, charts, logs and case/progress notes per the instruction of the Direct Supervisor and the individual treatment plan methodologies.
* Maintain an orderly, clean, safe and habilitative core room environment at all times and address and/or report environmental deficiencies to the direct supervisor.
* Model appropriate behavior, appearance, interpersonal relationships and communication whenever in the presence of program participants.

QUALIFICATIONS:
* Associates Degree in Human Service Field and experience working with the developmentally disabled; OR
* High school diploma and one years experience working with developmentally disabled; OR
* A combination of education and experience that meets the needs of the reviewing agencies.
* Must be able to lift 50 pounds and not have any lifting restrictions which would prohibit him/her from assisting with toileting, transfers, physical intervention, lifting, etc.
* Possess a valid driver's license and good driving record acceptable to the agency's insurance carrier.
* Maintain certification in First Aide, CPR, SCIP and Medication Administration.
* Responsible for following agency safety and health policies, procedures and rules.
