As a part of our continued growth, we are searching for a Patient Care Coordinator to join our team.
 
Primary Responsibilities:

Works closely with the RN Case Manager to provide care coordination for ACO population
Documents care coordination activities in on-line systems
Checks benefits and available resources
Manages telephone calls, both inbound and outbound, for ACO Care Team
Communicates with ACO members, caregivers and providers
Follows all processes and procedures, including HIPAA

Requirements


Required Qualifications:

High school education or equivalent experience
Excellent interpersonal skills
Ability to work on a team and work independently
Medical terminology
Excellent written and verbal communication
Ability to read and interpret documents and write routine reports and correspondence
Ability to speak effectively
Ability to solve problems and deal with a variety of variables in a rapidly changing environment
General knowledge of managed care and regulatory requirements
Computer knowledge and proficiency to navigate various databases
Proficient in use of Microsoft Office applications: working knowledge of Outlook, Word and Excel
Proficiency with data entry
Able to multi task
Preferred Qualifications:

Experience in case management
Experience with ACO
