About this job

Providence is calling a Licensed Clinical Social Worker to the Providence Cancer Center at Providence Alaska Medical Center in Anchorage, AK.

The Licensed Clinical Social Worker provides services to patients and their families to meet their medically related social and emotional needs as they impinge on their medical condition, treatment, recovery, and safe transition from one care environment to another.

In this position you will:

Evaluate patients and their related support systems with a complete psychosocial assessment; use social work theory principals and methods to formulate and implement a psychosocial plan of action based on the patient needs and strength

Demonstrate knowledge and expertise in the assessment and treatment of substance abuse, mental capacity and incapacity, mental illness, adult protection, child protection, domestic violence, sexual and physical assault, psychosocial crisis, bereavement, and mental illness

Demonstrate knowledge and expertise in service areas and issues related to the continuum of care such as: adoption, advanced directives, public assistance and entitlement programs, disability and public assistance eligibility, community based waivers, guardianship, involuntary commitments, costs related to community programs, prescription assistance, public housing, homeless services, foster care, assisted living programs, and palliative and end of life

Identify and offer the necessary services, resources and opportunities needed for the patient's safe discharge to the community in collaboration with the Nurse Case Manager and interdisciplinary team

Maintain counseling skills needed to improve the developmental, problem- solving, and coping capacities of the patient or their surrogates; provide key education needed to address psychosocial concerns, and community resource needs for the patient or their surrogates

Follow hospital, Social Work, and Federal standards for the privacy and confidentiality of patient information and maintain appropriate documentation for patients per department guidelines

Use knowledge and skills necessary to implement appropriate care/services to the age and/or diversity specific needs of the patient/family

Supervises social work students from UAA or other social work program with Manager

Assists with orientation of new hires

Required qualifications for this position include:

Master's degree as a Social Worker from an accredited School of Social work

Alaska Licensed Clinical Social Worker (LCSW)

Two (2) or more years of Social Work experience

Excellent Communication Skills

Organizational and problem-solving skills

Preferred qualifications for this position include:

Two (2) or more years of Medical Social Work experience

Knowledge of Alaska community resources

Prior experience using Electronic Medical Records (EMR)
