Job Description
The LPN provides basic nursing care to assigned patients under the direct supervision of the registered nurse. The LPN collects data through observation and communicates information to assist the RN/Provider in patient assessment and care planning. The LPN is expected to deliver care in accordance with the patient care plan and standards of care. The LPN accepts delegation from the RN/Provider in meeting learning/information needs of the patient/family.
Basic Qualifications:
Currently licensed or eligible for licensure as a Licensed Practical Nurse in the state of Wisconsin, Minnesota or Iowa, depending on where they are employed. Basic Cardiac Life Support for healthcare providers- BCLS -- certificate of course completion required and must not expire within 3 months after start date. A scanned copy of the certificate of completion or transcripts, including your full name, must be attached to your profile to be considered.
Other Qualifications:
One year LPN experience within the last five years preferred. Excellent communication skills (verbal and written). Experience working in a team environment. Computer skills required, prior experience with electronic medical record systems preferred. Ability to work flexible hours, which may include days, evenings, nights, holidays, weekends, and on-call. Ability to adapt to unpredictable situations within the work setting. Graduate of an accredited school of practical nursing preferred.
License or Certification:
Current LPN license by applicable state requirements. Maintains BLS (Basic Cardiac Life Support) competency. Additional specialty certification/training as required by the work area.

Exemption Status: Non-exempt

Schedule Details: Must be available Monday through Wednesday 7:30-7 and Thursday/Friday 7:30-5
