About this job
Assists in meeting the psychosocial needs of patients and enabling patients to utilize medical care in order to achieve their optimal level of health. Assesses patients for discharge planning and assists with referrals to appropriate local/state/community resources. Educates patients, families, significant other regarding options for care outside the hospital as appropriate including but not limited to nursing home placement, home health, hospice, rehab, long term acute care, assisted living and DME needs. Assists with filing applications for financial aid. Assists in adoption of infants on the Women's and Children's unit per organizational policy when request received. Works closely with victims of abuse and/or neglect, makes appropriate referrals to the Department of Human Resources and provides appropriate resources per departmental policy. Assists with DHR guardianship when needed.EDUCATION/LICENSURE/CERTIFICATION/SKILLS REQUIREMENTS REQUIRED: Graduation from an accredited school of Social Work with a minimum of a Masters degree and current licensure in the State of Alabama required. Licensed Graduate Social Worker (LGSW) or Licensed Certified Social Worker (LCSW) required. EXPERIENCE PREFERRED: Case Management experience preferred. Familiarity with local referral agencies preferred. Working knowledge of Medicare, Medicaid, Blue Cross and other third party payers preferred. EXPERIENCE REQUIRED: Excellent communication skills (oral and written) required. Continuing education courses/competency validation in areas which update knowledge and expertise in discharge planning, financial assistance and community resources required. Working knowledge of Social Work theory and practice required.
