DescriptionTo perform this job successfully, an individual must be able to satisfactorily perform each essential function listed below.
_Develops and assures that a participant's program meets his/hers needs optimizing his/her potential to be integrated into community.
_Organizes and chairs program participant conferences.
_Fulfills a liaison role for families along with staff and provides them with information and resources as requested.
_ Responsible for development, coordination, carry-through of and follow-up on discharge planning as directed.
_Attends regularly scheduled supervision meetings.
_Reviews with designated program participants and assists him/her as needed to understand overall goals and programs being implemented in the process as requested.
_Promotes internal communication to team members relative to case-specific issues.
_Attends clinical team meetings as required.
_Reviews designated program participant's reports (evaluation, progress, and discharge) for consistency and coordination of goals.
_Reviews monthly bills for designated program participants after receipt from accounting staff. Identifies any discrepancies.
_ Ensures the appropriate documentation/record keeping of designated program participants.
_Ensures the provisions of appropriate services to designated program participants.
_Coordinates program participant’s incentive accounts.
_Performs other related duties and activities as required.

Requirements:
- Bachelor’s degree in Human Services or equivalent in education and experience required
- In addition, two years of related work experience and knowledge of case management
- Other education and experience plus licensure as required by state
