The Nurse Practitioner (NP) functions in advanced nursing utilizing written collaborative practice
agreements with physician(s) to manage medical and nursing care needs of patients in clinic
facilities. In addition, serves as clinical/educational resource for patient consultation.
This person performs the duties of the Registered Professional Nurse and Advanced Practice
Nurse consistent with established standards of practice, the goals of St. Johns and the
Employee Code of Conduct.
Performs related duties as assigned.

Responsibilities:
Develops and implements collaborative practice with physician(s) to effectively manage
medical and nursing care.
1.1 Performs and documents appropriate physical, mental, functional, and social
assessments.
1.2 Initiates appropriate referral for services based on nursing assessment.
1.3 Provides health teaching to patients and care providers.
1.4 Works with physician(s) to complete routine and episodic and consult visits for
nursing home residents.
1.5 Serves as patient/family advocate and liaison for clear communication of needs
and concerns.
2 Acts as a resource person providing consultation and referrals within the organization
and the community.
2.1 Serves as a role model for advanced practice nursing.
2.2 Shares professional expertise in formal and informal situations.
2.3 Acts as a preceptor for students in training for advanced practice roles.
3 Utilizes QI process to monitor care delivery for quality and cost effectiveness.
3.1 Identifies aspects of care and indicators for monitoring QI specific to the impact of
advanced nursing practice on health care delivery to older adults.
3.2 Develops and implements appropriate action plans.
3.3 Completes QI reports as requested.
4 Demonstrates willingness to continually update knowledge and skills.
4.1 Completes continuing education offerings appropriate to position needs and
national certification requirements.
4.2 Maintains membership in professional organizations.
4.3 Attends appropriate inservice offerings.
5 General duties.
5.1 Observes universal precautions consistently.
5.2 Wears proper personal protective equipment when appropriate.
5.3 Wears clothing and Health System badge consistent with dress code, and attends
to personal hygiene to maintain a clean, well-groomed appearance.
5.4 Meets Health System attendance standards.
5.5 Performs additional duties as directed.
6 6.1 Did employee, collaborative physician(s) and practice manager review
collaborative practice agreement?
Qualifications:
QUALIFICATIONS MINIMUM REQUIRED
Education: Graduate of an accredited school of nursing. Completion of an
accredited Nurse Practitioner program, certificate or Masters
level.
Licensure: Licensed as a Registered professional Nurse in the State of
Missouri.
Experience: Must have demonstrated clinical expertise in his/her area; and
an ability to excel in communication, human relations, and
organizational skills.
Certification/Registration: Certification or pending certification by the American Nurses
Association as an Adult or Family Nurse Practitioner.
Working Conditions, Mental and Physical Requirements:
Requires the individual to be self-motivated and be able to work
independently. Physical demands vary according to daily tasks.
Requires travel to outlying community and the physical demands
associated with clinical nursing.
