We focus on Integrity, Compassion, Relationships, Innovation and Performance as we empower people to achieve better health and well-being.As a Community Health Worker (CHW), you will act in a liaison role with Medicaid members to ensure appropriate care is accessed as well as to provide home and social assessments and member education.This position requires ACHA Level II Background checks (fingerprinting) by the State of Florida for all Clinicians that have face-to-face contact with members and will require renewal every five years.Primary Responsibilities:Create a positive experience and relationship with the memberKeep the member out of the hospital by supporting regular visits to their primary physicianKeep member actively engaged with their primary physicianSupport the member to ensure pick-up of their RxProactively engage the member to manage their careProvide member educationSupport transitions of careHelp to keep members compliant with their care plansPartner with care team (community, providers, internal staff)Knowledge and continued learning of community cultures and valuesMay conduct HRA assessments if neededThis is a field-based role, extensive travel in assigned area is required. 

Requirements

Required Qualifications:High School Diploma or GED with 3+ years of experience working within the community Health setting in a Healthcare roleKnowledge of culture and values in the local communityKnowledge of FL Medicaid populationFamiliarity with the resources available within the communityField Based experienceMust be comfortable with use of dual screens and multiple applicationAccess to reliable transportation that will enable you to travel to client and/or patient sites within a designated areaAHCA level II background checks (fingerprinting), required for all clinicians that have face to face contact with membersMust live in Miami-Dade County, FL. Preferred Qualifications:Bachelor's Degree in Social Work, LPN (Licensed Practical Nurse) licensure, CNA/HHACommunity Health Worker (CHW) AccreditationExperience in Healthcare or Health Insurance Experience working in Managed CareBilingual fluency with English & SpanishCareers with Optum. Here's the idea.
