ADMISSIONS NURSE

Interlude Restorative Suites West Health campus is located in Plymouth, MN. We are seeking a FT ADMISSIONS NURSE with a LPN licensure to support our Admissions team. We are a 50 bed transitional care unit that has a quick turn-around with an aggressive clinical and rehabilitation program.

This role is responsible for providing high quality patient/resident care services and to increase their satisfaction by facilitating and managing all aspects of the nursing admission process including assessment of patients/residents upon admission for appropriateness of admission, coverage and level of care determination: communication with internal staff regarding coverage and contract details; proatively work with multidisciplinary team to manage care, costs and discharge planning; and utilization review for all payer types, including Medicare, Managed Care and other third party payer systems.

Previous admissions/or intake experience is preferred. Long term care, acute care or subacute experience preferred. Knowledge of MDS, payer systems and contracts (Medicare, HMOs, Managed Care, Third Party Payer Systems) needed.

Responsibilities

The Licensed Practical Nurse (LPN) is responsible for the provision and monitoring of high quality clinical care and services to residents consistent with regulations and established clinical best practices.

Qualifications
  • Licensed Practical Nurse with current licensure with the State Board of Nursing in state(s) in which he/she practices.
  • Current CPR certification.
  • Interest in gerontological nursing, rehabilitative and restorative clinical practices.
  • Demonstrated compatibility with PHS's mission and operating philosophies.
  • Demonstrated ability to read, write, speak and understand the English language to communicate with all customers.
