Description REM South Central Services, located in Maple Grove, is seeking a Program Coordinator (House Supervisor) to provide assistance to three adult men living in a community-based setting. T he Program Coordinator is critical to our success and exemplifies the wonderful mission driven work we do here every day. The Program Coordinator will lead a team of caregivers on site in providing and managing the direct care of our customers in a group home setting. Program Coordinators facilitate the implementation of Individual Service Plans (ISP) and participate in a range of administrative duties including staff hiring, training, scheduling and oversight. In addition, Program Coordinators may provide direct support to individuals receiving services, including: assistance with daily activities such as meal preparation and planning; medication administration; and personal care. Program Coordinators may also facilitate community involvement by accompanying individuals on outings in the community or providing job training and transportation to work, therapy, medical appointments or other activities. Qualifications Requirements: High school diploma or Equivalent Must be 18 years of age Valid driver’s license in good standing and access to a registered vehicle with proof of insurance Ability to work inside and outside of the metro area and willingness to drive Successful clearance of local and/or state background checks Preferred Skills: Two years of full time work related experience in human services with one year of professional experience working with individuals living with a mental health diagnosis or willingness to learn how to successfully support our customers the individuals we serve Professional experience, education or acknowledgement of the criminal justice system Supervisory experience preferred The willingness and flexibility to learn to work within a new and innovative service model Founded in 1980, The MENTOR Network is a national network of local human services providers offering an array of quality, community-based services in more than 30 states to adults and children with intellectual and developmental disabilities, brain and spinal cord injuries and other catastrophic injuries and illnesses, and to youth with emotional, behavioral and medically complex challenges as well as their families.
