Description REM South Central Services, located in Maple Grove, is seeking a Program Coordinator (House Supervisor) to provide assistance to three adult men living in a community-based setting. T he Program Coordinator is critical to our success and exemplifies the wonderful mission driven work we do here every day. The Program Coordinator will lead a team of caregivers on site in providing and managing the direct care of our customers in a group home setting. Program Coordinators facilitate the implementation of Individual Service Plans (ISP) and participate in a range of administrative duties including staff hiring, training, scheduling and oversight. In addition, Program Coordinators may provide direct support to individuals receiving services, including: assistance with daily activities such as meal preparation and planning; medication administration; and personal care. Program Coordinators may also facilitate community involvement by accompanying individuals on outings in the community or providing job training and transportation to work, therapy, medical appointments or other activities. To that end, the Program Coordinator role is a fantastic role to continue your career path in Social and Human Services with a myriad of career options to move up and across the organization.
