This is a field based position with a home based (telecommuting) office. You will work from home when not in the field. This is a Monday – Friday, daytime hours position. Candidates will travel in and around the Lewiston, ID area.

The overall purpose of the Field Care Coordinator (FCC) is to coordinate care with members in order improve the member’s ability to remain stable in the community and out of the hospital. This coordination includes intensive coordination, discharge coordination, and care coordination for members. The FCC role is to engage the person in the treatment process and then assist his/her to access the appropriate community services so they can pursue their treatment goals in the community and avoid institutionalization. This position focuses on members with high needs, high utilization, and in need of complex Behavioral Health Care Coordination and works closely with members, providers, State stakeholders, Utilization Management, and Peer Support.     
The Field Care Coordinator position is also designed to be a direct clinical liaison between providers and the health plan and works in conjunction with the Quality and Network team to assist providers with technical support, including education in clinical managed care and education in clinical best practices.    
     
     
     
     

Requirements

Required Qualifications:  


Independent Licensed Master's degree in Psychology, Social Work, Counseling or Marriage or Family Counseling, or Licensed Ph.D., or an RN with 2 or more years of experience in behavioral health
License must be independent, active, and unrestricted
2 + years experience in a related mental health environment
Proficient computer skills and good working knowledge of Microsoft Word, Outlook, and Internet
Access to reliable transportation that will enable you to travel to client and/or patient sites within a designated area
Access to high speed internet services
Dedicated space for home office set up
Preferred Assets:  

Managed care background
Care coordination experience
Understanding of Idaho Health system
Proficient in working in member electronic medical records programs
Excellent customer service skills
Excellent interpersonal and problem-solving skills
Function independently and responsibly with minimal supervision
Strong team player and team building skills
Strong interpersonal skills
Maintain direct and open communication with all levels of the organization
Able to handle sensitive issues with members and providers in a confidential manner
Demonstrates initiative in achieving individual, team, and organizational goals and objectives
Ability and flexibility to assume responsibilities and prioritize tasks in a constantly changing work environment
Strong oral and written communication skills
