Hospice RN - 32 hours per week - Optum Hospice, Cincinnati OH - Full-time / Part-time

We are seeking a knowledgeable, compassionate RN to conduct patient visits. The schedule is 32 hours a week Monday- Thursday 8am-5pm. There will also be a required rotating on-call to participate in.
 
As a  Hospice Nurse, you would work with a team of healthcare providers to ensure our Hospice patients (prognosis of 6 months or less) are receiving the right care. Additionally, you would provide emotional support to the patients' families. This position offers potential for growth, a friendly staff, and an opportunity to interact with patients and their families on a more personal level.
 
Please note: This position is for a RN working 32 hours a week, field based position that requires travel throughout our service area (60 mile radius around our office in Cincinnati). On call and / or after hours triage work will be required as requested. Participation in monthly team meetings as well as quarterly all staff meetings is required.
    
Primary Responsibilities:

Conducting routine and admissions visits in the home and / or facility setting
Coordinating, assessing, and implementing patients' care
Providing accurate and timely documentation of patient services to reflect plans of care
Interacting with other group members such as CNA's, LPN's, social workers and dieticians
Performing physical activities that require considerable use of your arms and legs and moving your whole body, such as climbing, lifting, balancing, walking, stooping, and handling of biohazard materials
Participating in on call/afterhours triage as requested / required
Participating in monthly team meetings as well as attendance to quarterly all staff meetings

Required Qualifications: 


RN licensure (current, unrestricted) in the state location of position
1+ years of Acute care experience
1+ years of Hospice experience
Ability to perform physical activities that require considerable use of your arms and legs and moving your whole body, such as climbing, lifting, balancing, walking, stooping, and handling of biohazard materials

Access to reliable transportation that will enable you to travel to client and / or patient sites within a designated area
Preferred Qualifications:

Hospice experience
Home care experience
Long term care experience
Case management experience
Acute care experience
Proficiency with electronic medical records (EMR) software
