Department Profile\:

Home Care Services provides home health care to clients in Orange, Seminole and Osceola counties. Florida Hospital Home Care Services is a highly skilled team generally treats geriatric patients utilizing the latest technology. We provide services out of our central offices localized in Orlando and have complete electronic medical record.

Work Hours/Shifts\:

Mon â€" Fri 8.m. â€" 5p.m. Alternating weekend and holidays

Job Summary\:

The Registered Nurse assesses, plans, implements, coordinates, monitors, and evaluates home care services to meet the needs of the patient, caregiver and family members through communication, education and community resources to promote quality and cost effective outcomes within their home and community. If functioning as a case manager, the registered nurse assumes responsibility for managing the patients care through to completion. Actively participates in outstanding customer service and accepts responsibility in maintaining relationship that are equally respectful to all.

Knowledge, Skills, Education, & Experience Required\:
  • Minimum of one year of nursing experience, two years preferred
  • One year medical, surgical or critical/special care experience
  • Possesses basic computer skills.
  • Demonstrates the ability to communicate effectively and possesses good organizational and time management skills.
  • Displays flexibility, is self-directed and motivated.
  • Ability to prioritize and follow responsibility through to completion.
  • Current CPR card.
  • Home health experience (Preferred)
Licensure, Certification, or Registration Required\:
  • Current, valid State of Florida license as a registered professional nurse
  • Current, valid State of Florida driver’s license
  • Current auto insurance
Job Responsibilities\:

Demonstrates through behavior Florida Hospital’s Core Values of Integrity, Compassion, Balance, Excellence, Stewardship and Teamwork as outlined in the organization’s Performance Excellence Program

  • Performs initial and ongoing comprehensive assessment of patients, (caregivers/family) to determine home care needs, including drug review, safety, pain management and OASIS assessments at appropriate time points. Assesses for sign of abuse and/or neglect.
  • Develops a care plan/discharge plan in collaboration with the physician and patient and chooses the appropriate clinical pathway to follow.
  • Notifies physician of any change in patient status. Revises Plan of Care as necessary.
  • Educates the patient and caregiver/family regarding disease process, self-care techniques, health promotion, rehabilitation and disease prevention based on Home Care/nursing diagnosis.
  • Supervises home health aides and licensed practical nurse in care provided.
  • Practices good time management on completion of responsibilities and maintains productivity.
  • Adheres to infection control policies and procedures.
  • Follows appropriate use of PPE. Performs hand hygiene and nursing bag technique according to policy.
  • Maintains adequate current medical supplies for patients and car stock, timely and cost effectively. Stores medical supplies in a clean safe environment.
  • Participates in agency quality improvement activities to improve organizational performance.
  • HRN - Functions as a case manager for Private Duty Division, assumes the responsibility for managing the patients care through to completion.
  • HRN - Completes case management audits every 30 days.
  • Performs other duties as assigned or directed to ensure the smooth operation of the department.
