The Wound Care Nurse is responsible for assessing skin alterations, developing plans of care, implementing orders and treatments, and evaluating progress of specialized skin and wound conditions.  The Wound Care Nurse evaluates new admissions, participates in weekly wound team rounds, documents status, insures notification of physician and family of changes, and provides education to nursing staff. The Wound Care Nurse identifies wound types, analyzes trends, and supervises nursing personnel to deliver nursing care and within scope of practice coordinates care delivery, which will ensure that patient’s needs are met in accordance with professional standards of practice through physician orders, center policies and procedures, and federal, state and local guidelines.

Wound care certification is preferred. Prior wound care experience is required.

Job Specific Details:Coordinate and lead weekly wound team rounds and processes, including rounds, measurements, interdisciplinary care plan review, treatment recommendations, physician orders, and required documentationCollect, analyze and provide pressure ulcer information to nursing staff and other healthcare practitioners Investigate and identify center practices and risk minimizing measures, including accurate completion of Braden risk scales on admission, implementation of preventive interventions Monitor timely implementation of preventive interventions for at risk patients, including new admissions, individuals with skin alterations and individuals at risk for development of skin alterations Maintain records of pressure ulcers and skin alterations including risk assessment, prevention and minimizing activities Coordinate completion of ongoing pressure ulcer and skin alteration tracking activities and implements measures to improve outcomes, including employee education Review pressure ulcer rates monthly providing recommendations to the Quality Assurance and Performance Improvement Committee Recommend education for employees and other healthcare practitioners to minimize and, or prevent center acquired skin alterations and pressure ulcersEducate newly hired nursing staff on Skin Practice Guide requirements and Skin Resource Library including wound assessments, preventive interventions, care planning, treatment selection options, documentation requirements and education resources available Collaborate with administrator, medical director or his/her designee, Director of Nursing and other members of health care team on Skin Practice Guide Evaluate availability of needed supplies for wound care and coordinate ordering with central supply Assist the Director of Nursing and medical director with case review for center acquired pressure ulcers Monitor changes in state and federal regulations and current industry-standard practices Provide monthly, quarterly and annual reports including recommendations for changes in center practice for the Quality Assurance and Performance Improvement CommitteeThis is a full time 40 hour a week position. Could be 4 ten hour days. Looking for someone who is able to work independently and also within a team. Looking for a candidate with strong documentation, and time management skills.
