About this job

Under the administrative and clinical supervision of the Director of Social Services and the clinical supervision of the Clinical Supervisor, the patient care coordinator records multidisciplinary treatment plans, reviews treatment plans with patient, parent/guardians and acquires appropriate signatures, facilitates commitment hearings, and assists therapists in the completion of substance abuse assessments as needed.

Bachelor's Degree in mental health field preferred; attend and participate in continuing educational programs designed to increase awareness of changes in profession. Associates Degree with a major in health or human services, social work, counseling, preferred.

A minimum of two (1) years experience working in the mental health field. Ability to express ideas in a clear, concise and legible form; ability to develop and maintain therapeutic relationships with patients; demonstrate verbal and written communication skills; ability to interface with all disciplines; ability to organize and prioritize multifaceted workload; knowledge of casework activities; exercise tact, diplomacy and good judgment.
