Complex Care Manager, Registered Nurse

The Complex Care Manager RN is responsible for ensuring the continuity of post-acute patient care by utilizing the appropriate resources for patients with multiple comorbid conditions within the parameters of established contracts and patients' health plan benefits. The role facilitates continuum of patients' care utilizing advanced nursing knowledge, experience and skills to ensure appropriate utilization of resources specific to the patients' needs. The Complex Care Manager RN performs care management functions telephonically, with possible on-site visits as the need arises, and serves as an advisor to the rest of the team. The Complex Care Manager RN works collaboratively with nurse practitioners, care managers, consulting and primary care physicians, and other facility-based health care professionals to form a care team. The Complex Care Manager RN helps optimize care through more effective practice and accelerates the patient's progress by ensuring efficient yet compassionate care.

Essential functions of the Complex Care Manager RN role will include:

* Serve as the care team's primary resource for patients with Congestive Heart Failure (CHF) & Chronic Obstructive Pulmonary Disease (COPD) related comorbidities

* Conducts prospective, concurrent, and retrospective review of active patient care telephonically, with possible on-site visits when assigned. Consult with other care management members where needed for patients with complex cardiac and pulmonary issues

* Coordinates treatment plans, discharge expectations, and meets with patients and patients' families to discuss care plans

* Coordinates provisions for discharge from facilities, including follow up appointments, coordination with patient's specialists, transportation, etc., in order to maintain continuity of care and adherence to the patients' treatment plan

* Communicates authorization/denial for services to appropriate parties. Communication may include patient (or agent), clinician, and/or facility administration

* Educates patients and/or families about preventive care, medical issues, and use of prescribed medical treatments and/or medications

* Collaborates with other clinical and administrative staff in a positive, constructive manner.

* Demonstrates flexibility when performing assignments in order to meet organization goals

* Demonstrates, by action and attitude, the standard of high patient satisfaction, treating each patient with respect

* Use, protect, and disclose patients' protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards

* Perform other duties and responsibilities as assigned or requested

* Completion of RN accredited program

* Current RN license in Pennsylvania; additional license in New Jersey preferred

* Minimum one year of recent clinical experience, preferably in ICU or CCU floors; 3 -5 years preferred

* Minimum of 3-5 year of case management, disease management, transition of care experience required

* Previous care management, disease management, utilization review, or discharge planning experience preferred

* Excellent organizational and planning skills

* Experience working telephonically or with geriatric populations preferred
