At Landmark, our interdisciplinary teams collaboratively manage our complex patient panels. These teams are led by Physicians, Nurse Practitioners, and Physician Assistants, with supporting care provided by RN Nurse Care Managers, Social Workers, Pharmacists, Behavioral Health and other employed team members.

 

This position supports the Nurse Care Managers across the team to ensure all care delivery activities are coordinated and provided timely and efficiently to support optimal care outcomes. The NCMA is responsible for supporting the referral authorization process in a manner that ensures timely, appropriate member care meeting all federal and local rules, regulations and guidelines. Provides assistance to providers, members and co-workers related to the request authorization process. This position does NOT make independent decisions as they relate to medical necessity or use clinical judgment to either authorize or deny services. 

Responsibilities
    • First line phone contact for members, family members, caregivers and clinicians related to care of members enrolled in the care program.
    • Gather all necessary clinical and administrative information related to providing care to members managed the care program.
    • Schedule, communicate and support all clinician and Interdisciplinary care team appointments
    • Coordinate care such as schedule members’ appointments; arrange transportation, etc. according to established procedures, protocols and timelines.
    • Ensure communication between providers, including PCP, specialists, members of the interdisciplinary care team, members, families and caregivers including:
    • Clinical information, Care Plan and updates to PCPs, specialists and all clinicians involved in the care of the member
    • Communicate member orders to vendors
    • Forward physician orders for signature
    • Notify pharmacies of new prescriptions and authorizations of refills as ordered and/or authorized by the House Calls Clinicians.
    • Participate in quality monitoring process
    • Demonstrates a high level of customer service behavior at all times
    • Patient Satisfaction Calls
    • Expedites emergency calls as needed
    • Attends regular staff meetings as scheduled
    • Maintain HIPAA compliance standards.
Qualifications
    • High School Diploma or GED required
    • Physician office experience helpful
    • Medicare experience desired
