The Focus Group Population Health Coordinator performs a variety of patient related activities to assist physician office personal and the care management team, including contacting patients to ensure timely follow - up, address patient needs, and works with market management to reduce total medical cost.

 
Primary Responsibilities:

Documents all patient interactions in a concise manner that is compliant with documentation requirements for Model of Care and Center for Medicare and Medicaid Services (CMS) regulations
Conducts in - bound and out - bound calls for program requirements including, but not limited to: patient scheduling, surveys / screenings, reminder calls, census management and distribution of materials to appropriate clinical personnel or patients
Schedules and coordinates patient transportation, follow - up physician appointments in all applicable markets, as needed
Completes timely data entry of in - bound and / or out-bound call contact information into software applications (Claims Database, Trucare, etc.)
Utilizes various reports to track patient referrals and scheduling no - shows for patients in collaboration with Care Management staff and Clinic staff
Provides clerical and / or administrative support to clinical staff and managers for special studies, projects and reports
Provides excellent customer service by serving as a resource to all internal and external customers
Maintains knowledge of all health plan benefits, network, CMS, regulations, health plan and / or MSO policies
Call High Risk Members weekly as designated by PCP
Maintain accurate utilization scoreboard
Assist with Medication Adherence program
Verify patients have been seen according to population health stratification list
Assist all patients with barriers
Ensure appropriate clinical information is communicated when patients are referred to Care Management, Disease Management, Palliative Care, Hospice and Social Work services
Performs all other related duties as assigned
Complete monthly patient satisfaction surveys

Requirements


Required Qualifications:  




High school graduate or GED equivalent
More than one year of related experience in a medical setting
Basic computer literacy
Knowledge of medical terminology
Good communication and customer service skills
This position requires Tuberculosis screening as well as proof of immunity to Measles, Mumps, Rubella, Varicella, Tetanus, Diphtheria, and Pertussis through lab confirmation of immunity, documented evidence of vaccination, or a doctor’s diagnosis of disease
Preferred Qualifications:


Current, nationally recognized Medical Assistant certification or registration  
Bilingual in Spanish
Microsoft Excel Experience

If the hired individual resides in Florida (office based or telecommuting) this position requires the AHCA Level II background check (fingerprinting) by the State of Florida for all clinicians that have direct face to face contact with members OR employees who will have access to confidential patient data and will require renewal every five years.
