Southwest Medical Associates (SMA) is a multi-specialty group of Physicians, Nurse Practitioners, and Physician Assistants consisting of over 200 providers, fourteen clinical locations including nine health care centers, five urgent care clinics and an outpatient surgery center. SMA offers patients compassion, innovation, and quality care throughout southern Nevada. SMA is headquartered in the greater Las Vegas, NV area.

The Nurse Practitioner will be assigned to provide home visits. An advocate for the patients, the Nurse Practitioner or PA will provide total care for the patients assigned in collaboration with the physician of record and patient care team. The Nurse Practitioner or PA will communicate and manage the patients' care across all care providers, family, and caregivers. The Nurse Practitioner or PA will ensure that timely, patient-centered, appropriate evidence-based medicine is provided. The Nurse Practitioner will also conduct comprehensive one-time assessments in the home or in a skilled facility for assigned members.
 
Primary Responsibilities

The Nurse Practitioner will be responsible for the total care of the assigned patient population as part of the with the Patient Care Team (PCT)
This position is accountable for identifying and developing innovative actions to meet the needs of patients from both the health care and psychosocial / socioeconomic dimensions of care; as well as taking action for provision of services to meet the patient needs
The Nurse Practitioner will be responsible as part of the PCT for patient assessments, risk management plan development, maintenance care, acute interventions within scope, procedures such as IV access and wound care (not all inclusive), documentation and patient education
The Nurse Practitioner will ensure that all pertinent and active medical conditions are documented in the medical record in a manner compliant with CMS
Nurse Practitioner  will participate in rotating call

Requirements

Required Qualifications

Active, unrestricted APRN license in the State of Nevada
Nurse Practitioner Certification as an Adult, Family, or Geriatric Nurse Practitioner from either the ANCC or AANP required, or certified Physician Assistant
1+ year NP or PA experience
Must be able to effectively communicate with elderly and chronically ill patients and families
Understanding of Medicare, Medicaid and Health Plan benefit structures beneficial
Comfortable working independently in a home-visit setting
Ability to draw blood in the home for lab procedures
Willingness to perform procedures for self and other provider's patients
Willingness to travel by car to patients' homes or to skilled nursing facilities in assigned locations
Knowledge of computers and cell phones
Familiarity with ICD-10 and CPT-4 coding
 
Preferred Qualifications

Bi-lingual or multi-lingual
