The majority of your work is in the field at patient’s homes. Hours are typically between 7a and 6p. Candidates must be willing to travel within Forth Worth, Grand Prairie and South of I-820.
 
Performs personal care, basic delegated tasks, and incidental homemaking to assigned patients under the direction of a registered nurse.
 
Primary Responsibilities:


Follows the Home Health Aide Assignment Care Plan
Completes Home Health Aide note during visit and turns in within 48 hours
Performs other tasks as assigned under the direction of the Registered Nurse
Recognizes emergencies and has knowledge of emergency procedures including CPR
Participates in the agency's endeavors for accreditation, licensing and professional recognition according to state, federal or CHAPs requirements
Participates in home care Performance Improvement Program activities. Participates in periodic review of clinical records
Personal care duties to include:

Assisting patient with bathing and skin care
Assisting patient to transfer into and out of bed
Caring for the patient's hair and teeth
Retraining patient in necessary self-help skills
Assisting in feeding the patient and promoting adequate fluid intake      
Performs basic tasks

Reading and recording temperature, pulse, and respiration
Observes, reports, and documents client status and the care or service furnished
Performing simple procedures as extensions of therapy services such as assisting with exercises, ambulation, and range of motion as directed by skilled personnel
Assisting with medications a physician specifically orders and that are ordinarily self-administered
Reports to RN supervisor any changes in patient's condition and needs
Assists patient in toileting and elimination
Uses safe transfer techniques and ambulation
Performs correct anatomical positioning
           
Performs household duties essential to health care at home to include:

Changing the bed
Light cleaning
Making arrangements to ensure safety and enable the patient to reach necessary supplies or medications
Laundering, as necessary, for the patient's comfort and cleanliness
Promoting the proper nutrition for the patient by purchasing the necessary food
Assisting with the preparation of meals and providing nutritional support
Washing the dishes and utensils during the visit
 
 
 
 
 
 

Requirements

Required Qualifications;

A minimum of one (1) year full-time experience in direct care in an institutional (hospital or nursing home) facility or in an agency setting within the last five (5) years; or satisfactorily completed a training and competency evaluation program approved by TDHS; or submit to the Agency documentation from the Director of Programs or Dean of a school of nursing that the individual is a nursing student who has demonstrated competency in providing basic nursing skills in accordance with the school’s curriculum; and
Be on the TDHS nurse aide registry with no findings against the Aide related to patient abuse or neglect or misappropriation of property
Reasoning Ability: Ability to read and draw valid conclusions.  Ability to accept direction and learn new tasks
Maintains certification in Basic Cardiac Life Support Course C. Familiarity with medical terminology and equipment
This position requires Tuberculosis screening as well as proof of immunity to Measles, Mumps, Rubella, Varicella, Tetanus, Diphtheria, and Pertussis through lab confirmation of immunity, documented evidence of vaccination, or a doctor’s diagnosis of disease 
Physical Demands

Ability to work in a stressful environment. Positive work environment is promoted for all agency employees
Position requires extensive periodic travel within the agency's service area
