POSITION TITLE: Licensed Practical Nurse
DEPARTMENT:Med Surgical/ 6 South
GENERAL DESCRIPTION:
Assists with the care of patients whose nursing needs do not require
constant professional nursing care, and to assist professional nurses
with care of the acutely ill.
MINIMUM QUALIFICATIONS:
A. EDUCATION, TRAINING EXPERIENCE: Must be a graduate of a recognized
school of practical nursing. Previous experience in an acute care
setting desired.
B. KNOWLEDGE, ABILITIES SKILLS: Knowledge of the nursing care required
for the adolescent, adult and geriatric patient. Must have successfully
completed course of pharmacology and demonstrate competency before
giving limited medications. Must have successfully completed a course in IV
initiation and therapy, and demonstrate competency prior to providing IV
therapy.
C. LICENSES, REGISTRATIONS AND CERTIFICATIONS: Licensed Practical Nurse
in Illinois. Current CPR certification is required.

JOB DUTIES:
RESPONSIBILITIES/TASKS:
I. TECHNICAL/ADMINISTRATIVE/CLINICAL: (Total Weight of 100)
Demonstrates competence in the skills, (processes, procedures, and
equipment) necessary to carry out assigned duties as identified below.
Demonstrated competence in recognizing and responding to patients
physical, mental, emotional and developmental needs should be
evaluated in relation to the patient populations served as appropriate
for each responsibility.
A. Demonstrates competence in contributing toward the accomplishment of
patient goals by providing care and carrying out nursing functions
as assigned by the Registered Nurse.
1. Completes assessment every shift and reports abnormalities to RN.
2. Assists in providing comfort measures, i.e. repositioning, support
measures, bathing, oral hygiene, and bed changes.
. 3. Provides assistance with body elimination, i.e. enemas,
suppositories, catheter insertion (female/male patients) and
catheter care.
4. Assists patient with physical activities, i.e. turning,
ambulating, assisting to chair.
B. Demonstrates cooperation with other members of the health care team
in coordinating the care of the patient's total needs.
1. Assists with admission, transfers, and discharge of patient
following established guidelines.
2. Assists with procedures on the unit, i.e. dressing changes,
spinal taps.
C. Demonstrates competence in maintaining accurate and complete records
while on duty.
1. Maintains accurate IO record.
2. Documents care given directly to patients.
3. Participates in the evaluation and updating process of the
patient care plan.
D. Demonstrates competence in recognizing and reporting abnormalities
in patient's condition to the assigned Registered Nurse in a timely manner.
1. Gives report to the assigned Registered Nurse per shift.
E. Demonstrates competence in performing procedures and treatments
as assigned by the Registered Nurse.
1. Demonstrates competence in the administration of age appropriate
care.
2. Implements age appropriate care in accordance with established
standards of care, policies, procedures, and state and federal
law.
3. Administers medication according to established Medication
Policy.
4. Monitor the flow rate of existing intravenous lines.
5. Regulating peripheral fluid infusion rates.
6. Observing sites for local reaction and reporting results to the
Registered Nurse.
7. Discontinue IV's following established guidelines as directed by
the Registered Nurse.
8. Add non-medicated solutions to the existing patent peripheral IV lines.
9. Monitoring existing transfusions of blood and blood products.
10. Performing site care and changing dressings on existing
peripheral lines.
11. Documenting intravenous procedures performed and observations
made.
12. Provides oropharyngeal suctioning and tracheal suctioning/care
when appropriate following established guidelines (excluding
patients during the immediate post-operative period).
13. Provide nasogastric/gastrostomy tube irrigation excluding
gastrointestinal surgical patients during their immediate
post-operative period.
14. Provides application of ice bags/collars, water mattresses and
K-pads under direction of a Registered Nurse.
15. Provides application of hot and cold compresses, sterile and
unsterile dressings following established guidelines.
16. Collects specimens/cultures (urine, sputum, stool, and wound)
as ordered. This includes communication via the computer.
17. Weighs patients using regular, in bed, or wheelchair scale in
an accurate manner.
