About this job

We are searching for a Nurse Practitioner for our team-based mobile Primary Care Practice for Seniors in Southern California.

Come join us to give team-enabled care outside the walls of clinic and typical schedule, deeper in time and relationship, caring for patients who can't easily access the care they need. Team with us in this next part of your career!

What Were About
We exist to eliminate the health obstacles that prevent seniors from achieving what matters most. And we believe it all starts in the home, where we deliver primary care that combines the science of aging, modern technology, and the warmth of the kitchen table so the people we serve can spend less time feeling like patients and more time living as their best selves.
Its an ambitious goal that requires a team built from extraordinary and like-minded people who are big-hearted, determined, and patient-centered problem solvers who thrive in team environments and who prioritize integrity, empathy and careful stewardship of the resources needed for great patient care. Doing the right thing means doing well by our patients - first, last and always.
In our physician-led care model, nurse practitioners partner closely with physicians to implement the care plan for each patient within a shared patient panel. Because we care for home-bound patients who otherwise have difficulty getting to the doctor, the majority of the work associated with this position will occur in the field, caring for patients in their homes.
