Job Summary
We are seeking a Care Coordinator to join our team. In this role, you will work collaboratively with providers helping to ensure individualized care plans are followed and carried out, ensuring each patient gets high-quality, individualized care.  Responsibilities include greeting and communicating with patients in a prompt, pleasant, and helpful manner in order to complete processes involved with fulfilling orders for surgery scheduling, diagnostic tests, medications, etc as determined in the Physician’s treatment plan. Familiarity with insurance verification, pre-certification and authorization is required. Other responsibilities include conducting general office duties such as answering phones, scheduling patient’s appointments, filing, message relay, ensuring general organization and maintenance of patient records. The ideal candidate is compassionate, highly organized with excellent attention to detail and knowledgeable about healthcare practices and procedures. 
 
Responsibilities 
  • Communicate professionally with patients, doctors, and healthcare staff
  • Coordinate with providers to complete and fulfill medical record requests by following guidelines and protocols set forth by the practice. 
  • Coordinates care with outbound referring facilities/providers and ensure request are submitted or fulfilled in an accurate and timely manner.
  • Collaborate with colleagues to ensure consistent and comprehensive patient documentation of relevant communication in the practice management and electronic health records system.
  • Process patient registration and coordinate chart preparation for upcoming visits by ensuring all required or requested paperwork is completed.
  • Maintain the confidentiality of all patient medical records and information in a HIPPA compliant manner.
  • Follow all practice procedures in the accurate maintenance of and filing of patient records, documents and forms.
  • Complete administrative tasks as requested and other administrative and clerical duties as assigned.
Qualifications
  • Previous experience as a Care Coordinator or in a similar position is preferred but not required.
  • Certification as a medical assistant is preferred but not required.
  • Previous experience as a Medical Scribe or in a similar position is preferred but not required.
  • Previous experience as a Medical Records Clerk or in a similar role is preferred but not required.
  • High school diploma/GED or equivalent at a minimum is required.
  • Knowledge of medical terminology and administrative processes.
  • Familiarity with information management programs, Microsoft Office, and other computer programs.
  • Ability to take detailed notes and accurate documentation
  • Ability to manage multiple projects or tasks and prioritize appropriately.
  • Highly organized with excellent time-management skills with attention to detail.
  • Ability to work in fast-paced situations and make sound decisions quickly.
  • Ability to work well under pressure.
  • Excellent interpersonal skills and high level of compassion
  • Strong problem-solving skills.
  • Strong verbal and written communication skills.
  • Strong computer and transcription skills.
