Personal Care Attendant

Provide personal attendant services that are necessary, required, or advisable for the proper care of any client assigned to his or her care by Peggy Day Enterprises, Inc., (hereinafter referred to as the “Client”). Employee’s duties are strictly limited to supervising, feeding, and dressing Client and assisting with personal care activities, EXCEPT 20% of the time, in which the employee may assist with other duties, including, but not limited to, light housekeeping duties. *Employee is not permitted to administer medications to Client. Employee’s duties include:

• Providing supervision and personal assistance for Client at Client’s residence;
• Responding to Client’s needs in a timely manner;
• Providing companionship for Client;
• Assisting with feeding, dressing, bathing and personal care as needed;
• Verbally reminding Client to take medications, if needed; and
• Other assistance with activities of daily living as directed.

Although Employer reserves the right to assign other duties to Employee in Employer’s discretion, Employee understands and agrees that he/she will not spend any significant amount of time performing duties for Client that do not involve Client’s personal care, supervising, feeding, bathing, dressing, and other activities of daily living. If Employee is spending a significant amount of his/her work time (20% or more) performing services that are not personal to Client as described in this Agreement, Employee agrees to promptly notify Sadie Kvenild at (831) 659-5887. Employee agrees that he/she will at all times, loyally and conscientiously perform the duties and obligations required of him/her by the terms of this Agreement.
