SUMMARY

Responsible for providing high quality in-home case management services to Veterans and families housed through the Rapid Rehousing & Shelter Diversion Program. The case manager assists clients in developing goals that lead to housing stability, monitors progress toward these goals, and provides structure, direction, and service linkage for clients in order to assist them in sustaining permanent housing and achieving their life goals. Qualified candidate must possess a clear understanding of the service delivery system and the resources available for veterans and their families.

PRIMARY DUTIES

  • Develops with each client a detailed Housing Stability Plan centered on goals that assist in maintaining stable housing. The plan will include specific goals established but not completed while residing at the shelter, as well as new goals centered around housing stability, an action plans and timelines for the following areas: Financial needs and Budgeting, Employment and Educational Services, Family Support Services, Landlord/Tenant Relationship and Community Living
  • Provide services including community outreach, acting as a liaison with tenants and landlords, and mediating complaints/concerns to proactively prevent returns to homelessness.
  • Provides services that include connecting Veterans to VA health care benefits or community health care services where Veterans are not eligible for VA care.
  • Act as a liaison between the SSVF grantee and the VA or community medical clinic and works with a population of Veterans with complex needs who require assistance accessing health care services or adhering to health care plans.
  • Provides individual life skills counseling and support in above areas.
  • Provides a minimum of weekly in-home visits with clients providing structure and direction.
  • Monitors and provides linkages for clients in the areas of health, mental health, education and employment, housing, mainstream benefits, and parenting assistance.
  • Process Temporary Financial Assistance
  • Collects monthly statistics and submits to Director as required.

SECONDARY DUTIES

  • Keeps updated on most effective means of accessing services available within the larger community and maintains an active data base for referrals and community partners.
  • Participates in registration and processing of clients via Rosie (HMIS).
  • Ensures that each participant agrees, understands and signs and is in compliance with the Participant Service Agreement.
  • Provides crisis intervention as and when needed.
  • Basic knowledge of resources in the community available for veteran population, especially services and programs offered by the VA
  • Assists the Director in representing the mission and interest of the program in accordance with St. Vincent de Paul of Baltimore to various groups, organizations, and service providers.
  • Assists with and attends special events held by the Center and St. Vincent de Paul as necessary.
  • Upholds the vision, mission and values of St. Vincent de Paul.
  • Attends all staff meetings and other meetings as directed.
  • Supports a welcoming and positive environment for volunteers and works with them effectively.
  • Other duties as assigned within the scope of the position.

QUALIFICATIONS

  • Bachelor’s Degree in Human Services, or related field, required.
  • 2 years successful experience in working with persons who are homeless and/or materially poor in an urban setting, preferably in a housing program.
  • Willing and able to work a flexible schedule (weekends and evenings).
  • Knowledge of and respect for confidentiality.
  • Ability to relate in a courteous, comfortable, non-judgmental manner with persons from all ethnic, socio-economic, and religious backgrounds.
  • Desire and ability to work as a team player.
