TGH Crystal River is designated as an Accredited Chest Pain Center with PCI (Percutaneous Coronary Intervention), and Accredited Heart Failure Center and is a Joint Commission Certified Primary Stroke Center. * Active FL. RN License * 2 years acute care experience and 1-2 years Case Manager (CM) Experience * CM experience preferred at an acute care hospital * Discharge Planning Knowledge * Facilities a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services to meet an individual's and family's comprehensive health needs through communication and available resources to promote quality, cost-effective outcomes. * Enhance the quality of patient management, maximize satisfaction, and promote cost effectiveness. $15,000 Sign-on Bonuses for FT Registered Nurse Candidates * Active FL.
