Habilitation Specialist

Job Description:  The function of the Habilitation Specialist is to provide support and training to the Home Manager and direct care staff. The Habilitation Specialist also serves as a liaison between the Home Manager and the QMRP.

Job Duties / Responsibilities (Not limited to those listed):

• Conduct monitoring observations to ensure proper implementation of each client’s Individual Program Plan (IPP).
• Complete quarterly written analysis of collected data.
• Provide in-service training/instructions to the facility staff and home managers as needed.
• Conduct monthly school observations.
• Assist QMRP with completion of IPP’s.

·         Ensure that all clients have the necessary adaptive equipment and that it is in good working order.

• Initiate and participate in investigations of abuse/neglect.
• Monitor administration of medications.

·         Complete appropriate documentation on each client.

·         Ensure that fire evacuation drills are completed as scheduled.

·         Advocate on behalf of each client.

·         Attending scheduled training courses and meetings.

·         Provide reports/issues given by Home Manager to QMRP.

·         Assist Home Manager and QMRP as needed.

·         Review incident/accident reports provided by the home manager.

·         Provide training to the Home Manager regarding the overall operation of each facility.

·         Ensure that medication is in the home for each PRN medication.

·         Make sure Physicians orders, meal charts, BM charts, weight charts, sleep charts and menses charts are kept current and UpToDate.

·         Behaviors will be tallied monthly.

·         Get Dr. renewal orders signed and updated.

Job Qualifications:

• Demonstrated awareness of multiple cultures and cultural impact of the delivery of service, knowledge of Cumberland County courts area, social service system and victim’s rights as necessary.
• Must be in good physical, mental and emotional health.
• Must be willing to improve abilities, take suitable courses offered in the local community and attend workshops related to management.

Requirements:

·         Must possess a high school diploma or its equivalent.

·         Knowledge of proper methods to train developmentally disabled individuals. 

·         Knowledge of crisis intervention techniques.

·         Knowledge of laws and regulations governing the operations of group homes for developmentally disabled individuals.

Supervision

·         The habilitation Specialist is supervised and evaluated by the QMRP per the supervision plan requirements.

Evaluation

·         The work performance evaluation will be the QMRP. Evaluation includes the specific duties and responsibilities of this position, plus employee attitudes and general working behavior. Formal evaluations will be performed annually but may be initiated at other times by the employee or the Supervisor.
