Northumberland County Area Agency on Aging (AAA) provides assistance to those in need, encourages older adults to work through problematic situations, and assists individuals to remain active and independent within the community. More specifically, this agency does the planning, coordinating, and monitoring services for county residents over the age of sixty.

DESCRIPTION OF WORK

As an Aging Care Manager 2, you will put your customer focus, problem solving, and organizational skills to good use by evaluating and developing care plans for each client's distinctive needs and levels of care. This includes providing information and referrals to clients and their families, making regular home visits, providing after-hour emergency intervention as part of our on-call system, and completing intake and referral services in may formats. You will excel in this position if you like variety because all of our clients and their families have unique individual needs and we must ensure their care plans are documented appropriately in our databases, and all necessary forms are completed and filed for them.

Work Schedule and Additional Information:

  • Full-time employment
  • Work hours are 8:30am to 4:30pm, Monday- Friday, with a 30-minute lunch.
  • Salary: In some cases, the starting salary may be non-negotiable.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS
Minimum Experience and Training Requirements:

  • Six months as an Aging Care Manager 1 or a County Caseworker 1; or
  • Successful completion of the County Social Casework Intern program; or
  • Six months of professional human services or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or
  • An equivalent combination of experience and training.

  • Hiring preference for this vacancy will be given to candidates who live within Clinton, Columbia, Dauphin, Lycoming, Montour, Northumberland, Schuylkill, Snyder or Union counties. If no eligible candidates who live within these counties apply for this position, candidates who reside in other counties may be considered.
  • You must be able to perform essential job functions.

  • You must pass a background investigation.
  • This position falls under the provisions of the Older Adult Protective Services Act.
    • Under the Act, a conditional offer of employment will require submission and approval of satisfactory criminal history reports, including but not limited to, PA State Police and FBI clearance.
