As a Community Health Outreach Specialist, you'll play a crucial role in engaging with our patient community, offering essential services like crisis intervention, program enrollment, housing, and treatment placement, and liaising with community resources. Your duties include meticulous documentation of all patient services and participation in community forums addressing critical issues like homelessness and mental health.


Key Responsibilities:

  • Ensure access to services by facilitating translation, interpretation, and transportation.
  • Identify community-based resources and assistance programs for communication to community members.
  • Maintain accurate records of clients served, services rendered, and follow-up needs.
  • Act as a cultural mediator between communities and health and human services.


Qualifications include a High School diploma or GED, with an associate degree in human services preferred. Ideally, candidates will have two years of experience in community-based outreach services and proficiency in English and Spanish. Strong familiarity with Sussex County communities and a valid Delaware Driver's license with a clean driving record are required. The role involves diverse work settings, including passenger transport vehicles, businesses, schools, churches, and health center offices, with exposure to environmental hazards and physical tasks such as lifting (up to 50 pounds) and prolonged standing or walking. Effective communication skills, both in person and over the phone, are essential.
