  • Conduct pre-screen eligibility process for Program applicants through referral, phone and walk-ins. Determine and follow eligibility guidelines according to Program policies and procedures.
  • Assist in completing Intake process, including but not limited to, financial verification, income for room & board, and all required supportive file documentation electronically.
  • Assist in providing all required documentation to outside agencies that support the beneficiary file (probation, parole, courts, etc.).
  • Assist in completing short form applications completed by alternate Program staff in support of 24/7 intakes.
  • Assist follow-up on applicants that are referred to detox as a part of their rehabilitation plan prior to admission.
  • Be knowledgeable of community referral agencies and can communicate intake policies and procedures.
  • Assist in entering, processing, and maintaining relevant applicant information in the online records database while ensuring online files are timely, accurate and complete.
  • Collect medications and add to electronic files according to policies and procedures in the online database and provide medications to the Resident Manager for storage.
  • Support the Intake Coordinator/DRS to gather statistical data from all program Staff.
  • Complete and document intake urinalysis with the assistance of Resident Management.
  • This position will require evenings and some weekends.

  • Other duties as assigned.
