Job Description:
The Rural Mobile Support Team’s Case Manager arranges and conducts individual and community outreach visits to connect individuals, their social networks, and communities to necessary resources. The Case Manager works as a member of a team with a Peer Recovery Specialist and acts primarily as a system navigator to connect people with the care they need, particular to their situation. The Case Manager is required to develop connections with diverse community partners and providers, and to work directly with participants who are impacted by behavioral health through a lens of cultural humility and harm reduction (allowable under the grant).

Qualifications:
• Bachelor’s Degree in related field and/or Delaware Certified Alcohol and Drug Counselor within the first 6 months, with preference for experience with ASAM criteria.
• Experience in leadership practices, case management, narrative writing, and understanding and interpreting laws, policy and procedures.
• Must be able to communicate clearly with clients and providers and have knowledge of motivational interviewing techniques.
• Valid Driver's license and ability to maintain fleet service driving privilege. • Ability to deal constructively with aggressiveness, hostility, and other emotional attitudes.
• Ability to maintain effective working relationships with other service agencies, public and private, as well as state and federal regulatory agencies.

Preference: Fluent in Haitian Creole or Spanish
